[Herald Economy, Senior Reporter Park Jong-il] With the nationwide implementation of the ‘Community Integrated Care System’ on the 27th, Yeongdeungpo-gu (District Mayor Choi Hokwon) will fully launch the ‘Yeongdeungpo-type Integrated Care’ project, which provides medical, nursing, and care services all at once.
The ‘Yeongdeungpo-type Integrated Care’ system supports residents who need care to live healthily in their usual environment, linking medical, nursing, and care services according to the district’s characteristics. It mainly targets seniors over 65 and people with severe disabilities who find daily life challenging due to physical decline, disability, or illness.
Focusing particularly on supporting the community reintegration of discharged patients, the district promotes the ‘Safe Discharge Integrated Care Project.’ Last month on the 24th, it established a system to pre-share patient health status and care needs through agreements with seven medical institutions in the area.
Based on this, the district quickly connects tailored care services, including healthcare, nursing, living/housing support, and disability support, to prevent readmission or institutionalization after discharge, helping patients live stably at home. Particularly, by linking with the ongoing ‘Healthy Home’ personalized meal support project, the district provides customized meals, considering the health status of the recipients, such as meals for diabetes, cancer patients, porridge, and side dishes, to support a healthy life.
Moreover, through the ‘Zero Care Gap, Care Package Project’ and ‘Healthy Smart Home Visit Exercise Project,’ care givers visit the homes of recipients to support daily living such as cleaning, laundry, and bathing, and provide direct education on exercises like stretching and health gymnastics.
In addition, the district improves the living conditions of care recipients. Through the ‘Zero Falls Home Care Project,’ it installs fall prevention items and facilities, such as removing thresholds, installing non-slip tiles, floor finishing indoors, adjusting the positions of switches and outlets, and installing fire detectors, to prevent falls and other safety accidents, enhancing convenience in daily life.
Also, aligning with the launch of the integrated care system on the 27th, they will hold the ‘Yeongdeungpo-type Integrated Care Project Vision Proclamation Ceremony,’ officially declaring a vision to provide residents with specialized services by organically linking diverse care resources unique to Yeongdeungpo-gu.
Households wishing to apply for the integrated care service can do so by visiting the local community center in their residential area or the Yeongdeungpo Southern Branch of the National Health Insurance Corporation. The application can be made by the beneficiary themselves, their family, or a guardian. After application, a visit survey and integrated assessment process follow to finalize the selection of the beneficiary.
Yeongdeungpo District Mayor Choi Hokwon stated, “The ‘Yeongdeungpo-type Integrated Care’ project is a community-driven care system,” adding, “We will put more effort into linking and providing services so that residents in need can truly feel the care in their daily lives.”
